Mobile Primary Health Care in Sai Kung Rural for Elders

Elderly residents and those entering old age in the rural areas of Sai Kung have significant needs for primary care services. Due to geographical and transportation limitations, they face difficulties in accessing mainstream healthcare services such as government clinics and nearby hospitals. The goal of the program is to establish the "first point of contact" for a healthcare system in the rural areas of Sai Kung, providing mobile primary health care services to residents. Through collaboration among interdisciplinary professionals in the medical and social sectors, the program aims to enhance citizens' self-management of health, improve residents' health and quality of life, alleviate caregivers' burdens, reduce hospital admissions, and achieve "aging in place" within the village.
The services provided include distributing smart devices to record health and exercise conditions, regular health assessments and lectures conducted by health coaches in the villages through phone or in-person consultations to assess or follow up on participants' health conditions, and providing mobile or home-based medical services, including traditional Chinese medicine, Western medicine, physiotherapy, and occupational therapy.
