The HKFYG Wellness PLUS Complex has introduced the "Social Prescribing" health management model, which provides personalised health improvement plans for service recipients troubled by emotional issues or obesity through non-medical intervention. The prescription includes health knowledge on emotional well-being, exercise, and nutrition, lifestyle modification, and social services or resource referrals to enhance holistic health. Through medical-social collaboration, mild or stable cases are transferred to community support to alleviate the burden on healthcare services. Social Prescribing emphasizes collaboration among various sectors, including medical, social, governmental, commercial, educational, and innovation. By promoting social participation, we connect stakeholders in the community to advocate the Social Prescribing health management model, actively promote medical-social collaboration, establish a solid foundation for programme development, and gradually enhance the health awareness of children and youth. Furthermore, we are committed to capacity building. Through professional knowledge training and community internship opportunities, we cultivate young individuals who aspire to become advocates for holistic community health, further promoting the programme's principles and sharing knowledge.
