Cancer affects countless families. At present, cancer treatment is mostly carried out in the form of day outpatient service which mainly focuses on the treatment of patients' physical symptoms. Psychological and social needs, family issues and financial difficulties of cancer patients are often ignored. Patients need to go home to recuperate after day treatment.  The responsibility of caring for patients falls on family members which puts them under great pressure. Cancer patients and their families felt frustrated and anxiety. 

 In view of this, the “Jockey Club Cancer Survivorship Care Project” was launched by the Hong Kong Anti-Cancer Society and funded by the Hong Kong Jockey Club Charitable Trust since 2011 to support cancer patients and their families. Our social workers/nurses reach out to provide personalised, family-based, community support for patients and their caregivers along different stages of the cancer continuum. Services include telephone counselling, home visits, hospital outreach support, integrative body-mind-spirit programmes, purchase of community services etc. We have served over 10,000 cancer patients till now. 

The service covers Hong Kong Island, Kowloon and the New Territories, providing patient-centred, comprehensive support to cancer patients and their families throughout the entire cancer journey.
